MILWAUKEE — As COVID-19 vaccines continue to roll out across Wisconsin, it can be hard to know where to turn to schedule an appointment.

As of April 5, every Wisconsinite who is 16 or older is eligible to get their vaccine. Here, we’ve collected information from vaccine providers to help you get your shot. 

Remember that vaccine supply is still limited, so you may not be able to get an appointment right away. And remember that 16- and 17-year-olds are only approved to receive the Pfizer vaccine — not the Moderna or Johnson & Johnson shots — and will need parent or guardian consent for the vaccine.

 

Statewide resources

On March 3, the Department of Health Services launched its online registry to connect residents with vaccine providers.

Eligible residents can use the registry to book appointments through several local health departments and community clinics, including: Madison & Dane County, Eau Claire, Green County, La Crosse County, Marathon County, Oneida County, Racine County, Rock County, Wauwatosa, and multiple FEMA mass vaccination clinics in Milwaukee. The DHS said more sites may be added over time.

Wisconsinites who aren’t up for their shots yet can also sign up to get notifications when they become eligible.

And for those who don’t have internet access or otherwise need help getting a vaccine, the DHS has set up a toll-free vaccine hotline at (844) 684-1064 to answer questions.

VaccineFinder — a site operated by Boston Children’s Hospital and supported by the Centers for Disease Control and Prevention — can also help you locate nearby vaccination sites, though you can’t book an appointment on the site.

 

Local health departments

  • Barron County: Eligible residents can complete an online survey or call 715-537-6123 for help from the county health department in connecting with a local vaccine provider.
  • Brown County: Lambeau Field is now open as a vaccination site run by Bellin Health and the local health department. Any eligible Wisconsinite can call (920) 445-7313 to add their name to the waiting list or schedule an appointment via the Bellin Health online portal

  • De Pere: The city health department is offering vaccine clinics. Eligible residents can sign up for appointments online or call 920-339-4054 to schedule over the phone.

  • Douglas County: Eligible residents can call the COVID-19 vaccine hotline at 715-395-7336 and leave their name with spelling, birthdate, and a phone number. Douglas County Public Health will call residents back when appointments are available.

  • Eau Claire: Eligible residents can check online to see if appointments are available with nearby providers or through the local health department. For scheduling assistance, call the COVID-19 Call Center at (715) 831-7425.

  • Fox Cities: Local health departments are holding a series of vaccination clinics at the Fox Cities Exhibition Center. Eligible residents can register here or call (920) 399-2550 if they do not have internet access.

  • Kenosha: Eligible residents can register online for a vaccination appointment at the old Shopko building, where the county is holding a clinic. Residents can also call the vaccine call center at 262-605-6799 for help scheduling a shot and check the health department website for other local options.

  • La Crosse County: The DHS is holding a community-based vaccine clinic at UW-La Crosse, located at the Cleary Alumni Center. Eligible residents should sign up through the statewide vaccine registry or call 1-844-684-1064 between 7 a.m. and 7 p.m. for assistance. 

  • Madison and Dane County: Vaccination clinics at the Alliant Energy Center are ongoing, with FEMA support as of Wednesday. Eligible residents can sign up through the statewide registry.

  • Milwaukee: Vaccination clinics at the Wisconsin Center are ongoing, and FEMA is assisting with other mass vaccination sites in the city as well. Eligible residents can sign up through the statewide registry, call the Milwaukee Health Department hotline at 414-286-6800, or call the state hotline at 844-684-1064. UW-Milwaukee is also running a community vaccination clinic, with signups available online

  • North Shore (Bayside, Brown Deer, Fox Point, Glendale, River Hills, Shorewood, and Whitefish Bay): The local health department is offering regular vaccine clinics. Eligible residents can book appointments online or call 414-229-4444.

  • Oneida County: Eligible residents can sign up through the statewide registry for appointments at the local health department’s vaccine clinics.

  • Oshkosh: The UW-Oshkosh campus is hosting a community vaccination site on its campus, located at the Culver Family Welcome Center. Eligible residents can sign up for an appointment online. The online form opens around 3 p.m. every Friday.

  • Racine: The DHS is running a community-based vaccination clinic at Regency Mall. Eligible residents can sign up using the statewide registry or call the state hotline at 844-684-1064. The Central Racine County Health Department and the City of Racine Public Health Department are also offering some vaccine appointments, which can be booked online.

  • Rock County: The DHS is running a community vaccine clinic in Rock County. Eligible residents can sign up through the statewide registry or call the state hotline at 844-684-1064.

  • Sheboygan County: Eligible residents can sign up online for appointments through the public health department or call 920-395-9890 for help scheduling.

  • South Shore (Cudahy, Oak Creek, South Milwaukee, and St. Francis): Health departments are offering a vaccine clinic in Cudahy. Residents can schedule appointments online, call the Oak Creek Health Department at 414-766-7950 for scheduling help, or sign up online to receive weekly email updates about the clinic.

  • Washburn County: The local health department is running vaccination clinics at the Washburn County Annex Building. Eligible residents can sign up for an appointment online or call 715-635-4402 for assistance. Appointments for each week are posted at 8:30 a.m. on Monday.

  • Washington and Ozaukee Counties: Eligible residents can sign up online to be added to the vaccine waitlist or call 844-684-1064 for assistance.

  • Waukesha: Residents can sign up online or text WAUKESHA to 22828 for weekly updates on the county’s vaccine rollout. 

  • Wauwatosa: The Wauwatosa Health Department is running regular vaccine clinics. Eligible residents can sign up through the statewide vaccine registry, the city health department, or call the local health department’s hotline at (414) 475-1050 for assistance.

 

Health care providers

  • Advocate Aurora: Patients 16 and older can schedule their appointments on the LiveWell online portal or call 866-443-2584 to schedule over the phone.

  • Agnesian HealthCare: Patients 18 and older can schedule a vaccine appointment online. For patients who are 16 or 17, a parent or guardian should call (920) 926-8400 to schedule an appointment.

  • Aspirus: Patients can request a vaccine appointment online through the patient portal. To request a vaccine with Aspirus Divine Savior Portage Clinic, patients can call 608-745-6490.

  • Bellin Health: Patients can schedule vaccine appointments online or call a COVID-19 hotline at 920-445-7313. Those younger than 18 should call to schedule.

  • Froedtert: Eligible patients will be contacted directly via MyChart, email, text, or letter to schedule an appointment.

  • Gundersen Health: Patients can schedule vaccination appointments through the online MyChart portal or call (608) 775-6829. Those younger than 18 should have a parent or guardian call to schedule an appointment.

  • Marshfield Clinic: Eligible patients can schedule an appointment online, attend a walk-in clinic, or call 877-998-0880 for assistance.

  • Mayo Clinic: Eligible patients will receive an email or letter to schedule an appointment when one is available. 

  • Prevea Health: Prevea is running several community vaccination clinics across the state. Patients and members of the public can use an online scheduling tool, or call 1-833-344-4373 for help getting an appointment. Patients who are 16 or 17 years old will need to call for scheduling.

  • ProHealth: Patients who have been seen by a ProHealth provider in the last two years can call 262-928-5566 to schedule an appointment.

  • SSM Health: All Wisconsinites 18 and older can schedule a vaccination appointment online. Those who are 16 or 17, or who otherwise need scheduling help, can call 608-250-1222 for Dean Medical Group appointments or 608-324-1815 for Monroe Clinic appointments.

  • UW Health: Eligible patients can schedule an appointment online or call (608) 720-5295 for assistance.

  • Veterans Affairs: Health care teams will contact eligible veterans directly to schedule appointments. Veterans can also sign up online to get regular updates from the VA.

 

Pharmacies

  • Costco: Eligible customers can sign up online for a vaccine appointment at several locations across the state.

  • Hometown Pharmacy: Eligible customers can sign up for a vaccine waitlist at many locations in Wisconsin.

  • Meijer: Eligible customers can complete an online registration form, text “COVID” to 75049, or call their local Meijer pharmacy to be notified about scheduling when shots are available.

  • Pick N Save/Metro Market: Eligible customers can register online for appointments.

  • Walgreens: Eligible customers can register online for vaccine appointments.

  • Walmart/Sam’s Club: Eligible customers can sign up online through the Walmart and Sam’s Club websites.
